The olecranon is part of the extensor mechanism of the elbow. It can be considered as analogous to the patella and treatment has some similarities.
Clinical
- Often results from a fall on the outstretched arm.
- Check for open wounds and neurovascular status.
- Check for integrity of the extensor mechanism. This may be difficult as the patient is in pain, but it may make the difference between conservative and surgical treatment.
- Look for other injuries.
Radiology
AP, Lateral and Oblique films should be reviewed.
Treatment
- undisplaced (<2 mm, stable): cast 3 wks (elbow in 45o flexion) then gentle ROM
- displaced: ORIF (cancellous screw or tension band wiring) and early ROM if stable
