Avulsion frctures of the 5th metatarsal in those who are neurologically intact (i.e non-neuorpathic) are best treated with a boot or comfortable shoe to minimize pain.  This allows the fracture to heal (or become asymptomatic) comfortably.  Only rarely does surgical treatment need consideration.  In rare cases where there is extension of the fracture into the 5th metatarsal-cuboid joint with angulation of the fragment causing pain surgical reduction and fixation of the fracture may be necessary.
