Should a subacromial decompression include a partial acromial resection or only a bursectomy and soft tissue acromioplasty?

    As I have argued elsewhere the word impingement is at best a misnomer--the bone, in most cases to be sure, is not pressing on anything it should not be pressing on. Or as it has been said (by Dr Matsen, I am told): "The acomion is innocent!"

    This question is asking for a well designed and well executed prospective double blinded trial: simply compare two groups of similar patients, one of which gets a bony acromioplasty as well. (I would love to see a third arm to the trial, namely a sham surgery placebo group, but I won't get greedy...)
